Documentation Guidelines

Students are asked to provide documentation from a qualified professional that establishes a clear and current understanding of your condition and how it impacts your academic experience.  

General Guidelines 

  • Documentation should be current, typically no older than five years, to reflect your current condition and its functional limitations. For permanent and/or unchanging conditions, older documentation can be accepted.

  • Documentation must be provided by a licensed and/or certified professional with expertise in the area of your disability. This may include a physician, psychology, psychiatrist, or other relevant profession and/or specialist. Documentation from family members is not sufficient. 

  • Documentation should be on letterhead, signed, and dated, and must provide the professional’s name, title, professional credentials, address, and phone number.  

Further Guidelines 

Documentation should include: 

1. A clear diagnostic statement with relevant diagnostic codes (DSM-5 or ICD-10) 

2. A summary of the diagnostic process, such as a history of the condition, a description of its symptoms, and the date you were either diagnosed, or seen by the professional.  

3. A detailed explanation of how your condition impacts your academic performance and participation in college life. This should explain the functional limitations created by the condition, and should generally connect the diagnosis to areas such as reading, writing, test-taking, concentration, or other daily activities. More than anything else, the functional impact is the most important component of documentation to help determine what accommodations might best support you at CCNY. 

If it is helpful, the Access/Ability Center has a Request for Disability Documentation Provider Form that can be completed by your provider or professional.
